Job Description: Roll out WBPHCOT plan to all identified areas and attend meetings [ads1] Job Responsibilities: Facilitate collaboration with other relevant stakeholders (ward councillors)/ Partners/ NGOs. Training, assessment, marking and moderation of outreach team leaders (OTLs) and Community health workers (CHWs) Manage, supervise, collate, verify&write the reports and regular support visits to team. Order material and manage resources allocated to the sub district for ward-based outreach teams. Report to the area manager and WBPHCOT district coordinators. Job Requirements: Proof of current registration, Basic qualification accredited with SANC (South Africa Nursing Council in terms of Government notice as Professional Nurse R 425 (i.e. Diploma/Degree in Nursing) with Community Nursing and registration with the SANC. 3 years of experience working in the Primary health care setting as a Professional Nurse. Should be willing to work in the community setting and have a valid driver’s license.
