Provide advice and guidance on housing options, including social housing, private renting, and supported accommodation.Develop personalised support plans for clients, outlining goals, actions, and time lines to address their housing and support needs. Collaborate with internal teams and external agencies to access relevant services and support.Implement homelessness prevention strategies, including mediation, financial assistance, and advocacy, to prevent evictions and secure housing stability for at-risk individuals and families.Make appropriate referrals and signposts to internal and external services, including housing providers, support agencies, healthcare services, and welfare benefits, to meet client's needs and address barriers to housing stability.Maintain accurate and up-to-date records of assessments, support plans, referrals, and outcomes using the organization's database systems.
