We require the services of a Community Social Worker that will work collaboratively with St Luke’s Combined Hospice’s Inter-Disciplinary Team (IDT) to provide psychosocial support to patients, their family members and significant others.ENTRY-LEVEL QUALIFICATION, KNOWLEDGE AND EXPERIENCEProfessional Social Work qualification. (BA Social Work, BSocSci, Diploma in Social Work)A minimum of 3 year’s related training and/or related experience in a similar position.Previous experience in a Health care settingExperience in palliative care and understanding its role in society will be an added advantageCertification in Palliative Care (added advantage).
