We require the services of a Community Social Worker that will work collaboratively with St Luke’s Combined Hospice’s Inter-Disciplinary Team (IDT) to provide psychosocial support to patients, their family members and significant others.ENTRY-LEVEL QUALIFICATION, KNOWLEDGE AND EXPERIENCEProfessional Social Work qualification. (BA Social Work, BSocSci, Diploma in Social Work)A minimum of 3 year’s related training and/or related experience in a similar position.Previous experience in a Health care settingExperience in palliative care and understanding its role in society will be an added advantageCertification in Palliative Care (added advantage). Those applicants who do not meet this criterion, will be expected to study towards the certificationCurrent registration with the SACSSPFluency in English and 1 other official languageIntermediate MS Office Suite literacy – Outlook, Word and Excel.Unendorsed driver’s license with long-term experience driving a manual vehicle.KEY PERFORMANCE AREASProvide holistic psychosocial assessment of patientsProvide counselling support to patients and familiesFacilitate material support for food security ,grant applications, wills, UIF and other psychosocial support as requiredPerform crisis interventions when necessaryCompile and submit reports e.g. Psycho-Social Assessments and patient StatisticsRefer patients to other specialised services within the organisation when so requiredMaintain all patient data in accordance with the requirements and guidelines of SLCHWork in an Interdisciplinary team approach to render intervention to patients and familiesAttend meetings and workshops for continuous professional development.
